Our overarching aim is to reduce the associated risks, increase your safety and promote your independence, in all your activities of daily living (we sometimes refer to these as Functional/Occupational Performance areas).

  1. Contact/Referral

Once contact has been made from an enquirer (client, family, case manager etc), we will need to gather some basic information to decide whether we can provide any service.  We would typically do this over the telephone, email or video conferencing. We have a standard referral form which guides our decision-making process

  1. Service Agreement

We will submit our Assessment and Interventions contract proposal with a breakdown of the Fees, Terms & Conditions, Consent Forms etc. You have time to decide whether our offer is accepted or not.  Once all documentation is returned to us, we will contact you to set up the initial assessment.

  1. The assessment usually takes place in your environment (home, school or work) as these are the unique settings for your functional occupational engagement. Currently we are limiting the time spent indoors so please refer to our Covid-19 page for further information,
  2. During the OT Assessment, we gather as much relevant information as we can. This in conjunction with various tests (formal & informal), our professional observational skills and our clinical judgement, informs our report and recommendations.
  1. Our report is written up within 5 working days of the assessment,
  2. It will contain your basic information, a general baseline about the individual’s functioning, results of various tests, our findings and recommendations,
  3. Our report and recommendations will guide future therapies and the goal development,
  4. The recommendations will outline our proposed treatment plan
  1. Once you have reviewed the Assessment report, we will sit down with you and come up with a detailed individualised treatment program,
  2. Treatment will commence once the program has been finalised,
  3. We will then review the efficacy of this treatment at suitable intervals to decide what happens next – further intervention or stop/cancel intervention (goals met, no longer needed or no satisfaction with the treatment.
